She brings over 20 years of nursing experience, including expertise in geriatrics, open-heart step-down, med-surg, telemetry, transitional care and nurse case management. NP Vazquez specializes in providing comprehensive, home-based primary care to adult and geriatric patients, with a strong focus on managing complex and chronic conditions. She is dedicated to delivering compassionate, patient-centered care while collaborating closely with healthcare partners to improve outcomes, reduce hospitalizations, and support patients and families throughout every stage of care.
